Join the practice
To register with our practice, you’ll need to complete:
- Our New Patient Questionnaire
- Our New Patient Registration Form
These are available at reception, or you can complete them online using the links below. You may wish to provide identification for registration with the practice but it is not required. We will not refuse to register someone because they cannot produce proof of identity or immigration status or proof of address. For more information on registration please see our registration form
Once your registration is accepted, your medical records will be transferred to us and you’ll receive a new medical card. Please note it can take several weeks for your records to arrive.
If you’re on regular medication
Book a 20-minute appointment with a GP soon after registering. This helps us ensure your ongoing care continues smoothly.
For all new patients
We recommend a routine health check with our practice nurse within your first 3 months.

Temporary Registrations
If you become unwell while away from home, or if you’re not registered with a GP but need to see one, you can get emergency treatment from a local GP practice for up to 14 days. After that, you’ll need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This means you’ll be added to the local practice’s list but will still remain registered with your usual GP. After three months, you’ll need to re-register as a temporary patient or join the local practice permanently.
To register temporarily, contact the GP practice you want to use. Practices don’t have to accept temporary patients, but they must offer emergency treatment. You can’t register as a temporary patient at a practice in the same area where you’re already registered.
